A non-CDL DOT physical medical card is issued to drivers operating commercial vehicles that do not require a CDL but still fall under DOT regulations. This exam ensures they meet the necessary medical standards for safe driving, including vision, hearing, and overall health evaluations.
A DOT physical consists of a thorough medical examination to assess a driver's overall health and ability to safely operate a commercial vehicle. It includes checks on vision, hearing, blood pressure, urinalysis, and a review of medical history to ensure compliance with DOT health standards.
For a DOT physical, you should bring a valid driver's license, a list of current medications with dosages, and any relevant medical records for existing conditions such as diabetes, heart issues, or sleep apnea. If you use corrective lenses or hearing aids, be sure to bring them, along with a CPAP compliance report if using a CPAP machine.
For a DOT physical, you will typically need to fill out a few forms, including the Medical Examination Report Form (MCSA-5875), which details your medical history and current health status. You'll also need to complete the Medical Examination Certificate (MCSA-5876) once the exam is finished. In some cases, additional forms may be required if you have specific medical conditions.
The price for a DOT physical is $79, and please note that it is not covered by insurance. We do not accept checks, but we do accept cash and major credit cards for payment.

Having a recent surgery does not automatically disqualify you from passing a DOT physical exam. However, the medical examiner may require Full Duty work documentation from your surgeon or physician confirming your recovery and ability to safely operate a CMV.
Yes. You do not need medical insurance to get a DOT Physical exam. DOT Physicals are considered occupational.
