A DOT physical exam is a health evaluation mandated by the FMCSA for commercial and non-commercial drivers. It ensures drivers meet the physical and mental demands of operating a motor vehicle, often required for employment, certifications, or other purposes. If the driver meets FMCSA standards, they will be certified with a DOT medical card


Maximum certification time is 2 years. Medical conditions such as high blood pressure, diabetes and heart disease are limited to 1 year. However, the certification period may vary depending on the results of the medical examination and any specific health conditions.


The DOT physical exam covers:

  • Vision Test: Minimum of 20/40 vision in each eye with or without corrective lenses.
  • Hearing Test: Ability to perceive a forced whisper at 5 feet.
  • Blood Pressure and Heart Rate: Ensures no hypertension risks.
  • Urinalysis: Screens your kidney for potencial medical conditions like diabetes.
  • General Physical Examination: Includes checking the lungs, heart, spine, and extremities.


No, it's a health screen checking for blood, protein and sugar. However, according to federal regulations under FMCSA 49 CFR Part 391, Schedule 1 drugs, including marijuana, are not permitted for commercial drivers—even with a prescription.


  • A valid driver’s license.
  • Bring your glasses or contact lenses if you use them for driving.
  • List of all your medications.
  • Any relevant medical documentation, such as:
    • Recent surgery requires a Full duty work clearance.
    • CPAP compliance data (for sleep apnea) 1 year compliance.

Having a recent surgery does not automatically disqualify you from passing a DOT physical exam. However, the medical examiner may require Full Duty work documentation from your surgeon or physician confirming your recovery and ability to safely operate a CMV.


Yes. You do not need medical insurance to get a DOT Physical exam. DOT Physicals are considered occupational.


One. You should have the condition, which caused the restriction, under control with in those 3 months.


 Dot physical examination takes around  25 to 30 minutes once you are in the exam room with the doctor.
