These ranges are commonly used to guide certification length for hypertension staging.

How to avoid delays

  • Bring your medication list (or bottles) and take meds as prescribed.
  • Avoid caffeine/nicotine right before your exam if possible.
  • Arrive early so you can sit quietly for a few minutes before re-check.
  • Hydrate and sleep — poor sleep can elevate readings.
  • If your BP runs high, ask your provider for a recent note showing treatment plan and stability.

Note: Blood pressure may be checked more than once during the visit, especially if the first reading is elevated.

What if my blood pressure is high at the exam?

Don’t panic — it’s common. Many drivers still certify, but the card length may be shorter and you may need follow-up.

Stage 1 (140–159 / 90–99)
Often results in a 1-year card. You’ll typically be asked to monitor BP and stay on treatment if prescribed.
Stage 2 (160–179 / 100–109)
FMCSA guidance allows a one-time 3-month card to start/adjust treatment. If reduced to ≤140/90, you may receive a 1-year card.
Stage 3 (>180 / >110)
Disqualifying until controlled. Once controlled to ≤140/90 and treatment is well tolerated, certification may be issued at 6-month intervals.

Can I pass a DOT physical if I take blood pressure medication?

In most cases, yes — if your blood pressure is controlled and the medication is well tolerated. Bring your medication list and any provider documentation if requested.

What if I’m nervous and my BP is high (“white coat”)?

Stress can raise readings. Arriving early, sitting quietly, and re-checking after rest may help. The examiner may take multiple readings.
