What Happens If I Fail a DOT Eye Exam?

Failing the vision portion of a DOT physical doesn’t mean you’re disqualified forever. Here’s what “fail” usually means, what happens next, and how most drivers get cleared quickly.

During a DOT physical, the medical examiner checks vision because it directly impacts safe driving. The “eye exam” portion is usually quick — but if your results don’t meet the minimum standard, the examiner may not be able to issue a certification that day.

What does “failing” the DOT vision test mean?

Most drivers who “fail” simply don’t meet the required vision standard at the time of the exam (often because they forgot their glasses/contacts, their prescription is outdated, or they’re having a temporary issue).

Good news: many drivers get cleared by wearing the correct lenses or getting an updated prescription and returning with documentation.

What happens next if you don’t meet the vision standard?

  1. You may have to come back with corrected vision.
    The examiner may say you need follow-up with an eye doctor (optometrist/ophthalmologist) before a certificate can be issued.
  2. If you meet requirements but have a condition that needs monitoring, the examiner may issue a limited card.
  3. You may need a vision specialist evaluation form.
    If the examiner needs confirmation of your vision status, they’ll tell you what documentation to bring back.

  1. Schedule an eye exam and get an updated prescription if needed.
  2. Return with your corrective lenses and any paperwork the examiner requested.
  3. Retest vision at the clinic and complete your DOT physical.

Can I still drive if I fail the DOT eye exam?

If you are not issued a valid medical certificate, you may not be medically qualified for safety-sensitive driving. Rules can vary by employer policy, but the safest approach is: don’t assume you’re cleared until you have a valid medical card.

How to avoid failing the vision test

  • Bring your glasses or contacts (and a backup if you have one)
