How Do I Get a Replacement DOT Medical Card?

Need a replacement DOT medical card? Contact the medical examiner who performed your DOT physical. If your exam was at HealthRoute, you can walk in for a replacement at no charge.

Losing or damaging your DOT medical card happens more often than most drivers think. The good news is that getting a replacement is usually simple — as long as you contact the medical examiner who performed your physical.

If you currently hold a valid DOT medical certificate and need another copy, here’s what you need to know.

Who Do I Contact for a Replacement Medical Card?

Your replacement DOT medical card must come from the medical examiner who originally completed your DOT physical exam.

Medical examiners are required to maintain records of the physical they performed. Because of this, only that examiner (or their clinic) can issue an official replacement copy of your certificate.

If your physical was completed at another clinic, you will need to contact that specific office directly to request a duplicate.

Important: The DMV, your employer, or a testing center cannot issue a replacement DOT medical card. Only the medical examiner who performed the exam can provide an official copy.

What If My DOT Physical Was Done at HealthRoute?

If your DOT physical was completed at HealthRoute, getting a replacement is easy.

You can simply walk in during normal business hours and request a replacement medical card. There is no charge for replacement cards issued from exams performed at our office.

We recommend bringing a photo ID so we can quickly locate your records and reprint your certificate.

Do I Need a New DOT Physical?

No — a replacement medical card does not require a new exam as long as:

  • Your current medical certificate is still valid
  • Your medical status has not changed
  • The original exam has not expired

If your card has expired, you will need to complete a new DOT physical to receive a new certification.

Tips to Avoid Losing Your Medical Card

Many drivers keep multiple copies to avoid problems during inspections:

  • Keep one copy in your wallet
  • Store a copy in your truck
  • Save a photo or PDF on your phone
  • Provide a copy to your employer for records

Having backups helps prevent delays during roadside inspections or employer audits.
