DOT Physical Hearing Standards: What CDL Drivers Need to Know

A lot of CDL drivers ask whether they can still pass a DOT physical if they wear hearing aids or have some hearing loss. The good news is that FMCSA allows drivers to meet the hearing standard with or without a hearing aid. If a hearing aid is required to qualify, that gets noted on the medical card. And just as important: you do not need an SPE certificate for hearing aids.

What is the DOT hearing standard?

FMCSA allows a driver to meet the hearing standard in one of two ways. The driver must either be able to first perceive a forced whispered voice at not less than 5 feet in at least one ear, with or without a hearing aid, or the driver must show an average hearing loss of 40 decibels or less in the better ear at 500 Hz, 1000 Hz, and 2000 Hz, with or without a hearing aid.

How the hearing test works at the DOT physical

Many drivers pass the hearing portion through the whisper test. During that test, the ear being tested is turned toward the examiner while the other ear is covered. The examiner uses a forced whisper from 5 feet away. If the driver does not pass that part, the examiner may require an audiometric test instead.

Helpful tip: Failing the whisper test does not automatically mean you fail the whole DOT physical. FMCSA guidance allows an audiometric test when needed.

Can you wear hearing aids and still pass?

Yes. FMCSA specifically allows the hearing standard to be met with or without a hearing aid. That is very important for drivers who function well with hearing assistance.

If you need a hearing aid in order to qualify, the medical examiner may place that condition on your medical certificate. In other words, your card can show that a hearing aid is required while driving. Just like corrective lenses can be noted for vision, hearing aids can also be noted as a condition of qualification.

Do you need an SPE for hearing aids?

No. This is one area that causes a lot of confusion. An SPE certificate is for certain limb impairments, such as missing or impaired arms, hands, legs, or feet. It is not for hearing aids.

So if a driver needs hearing aids, that does not mean the driver needs an SPE. If the driver meets the hearing standard with hearing aids, the driver may qualify normally with that condition noted on the card.

Important distinction: Hearing aid use is not an SPE issue. If a driver does not meet the hearing standard, the separate FMCSA pathway is a hearing exemption, not an SPE.

What if you do not meet the hearing standard?

If a driver cannot meet the standard through the whisper test or audiometric testing, that does not automatically end the conversation forever. FMCSA has a separate hearing exemption process for eligible drivers who do not meet the hearing requirement under the normal standard.

That is different from simply wearing hearing aids. A driver who passes with hearing aids does not need an exemption just because the driver uses them.

  • Bring your hearing aids if you use them for everyday driving
  • Make sure your hearing aids are working and have fresh batteries or are fully charged
  • Tell the examiner if you normally drive with hearing aids
