Do DOT Examiners Submit the Long Form to FMCSA?

HealthRoute Compliance Team

Many CDL drivers ask an important question after their DOT physical: “Does the examiner send my long form medical paperwork to FMCSA?” The short answer is no. The long form is part of your medical record and is not submitted to the Federal Motor Carrier Safety Administration.

DOT long form medical examination report and medical card explanation
Quick answer: The DOT examiner does not submit your long form medical examination report to FMCSA. Only the medical certification information from the medical card is transmitted electronically.

What is the DOT long form?

The long form is officially called the Medical Examination Report. This document contains the detailed medical information from your DOT physical.

It typically includes information such as:

  • Medical history
  • Medications
  • Physical exam findings
  • Blood pressure
  • Vision and hearing testing
  • Urinalysis results
  • Medical examiner notes

Because this document contains personal health information, it is treated as part of your medical record.

What gets submitted to FMCSA?

After the DOT physical is completed, the medical examiner submits medical certification information electronically through the National Registry system.

This is what allows the driver’s medical certificate status to be updated with state licensing agencies.

Information typically transmitted includes:

  • Driver identification information
  • Certification status
  • Expiration date of the medical card
  • Medical examiner information
The long form itself is not sent to FMCSA.

Why the long form is not submitted

The long form contains detailed health information protected under medical privacy rules. Because of this, it is maintained in the medical examiner’s records rather than submitted to a federal database.

This protects driver privacy while still allowing certification status to be verified.

How drivers can get a copy of their long form

Drivers are allowed to request a copy of their medical examination report.

If you need your long form, you can:

  • Contact the clinic that performed your DOT physical
  • Request a copy of your medical examination report
  • Stop by the clinic to pick up a printed copy

At HealthRoute, drivers can call our office or stop by if they need another copy of their paperwork.

What drivers should keep after the exam

After a DOT physical, drivers should keep the following documents:

  • DOT medical certificate (medical card)
  • Any supporting medical documentation
  • A copy of the long form
Tip: Keep your medical card in a safe place and store your long form with your personal records. If you ever lose it, contact the clinic that completed your exam.
