Do DOT Examiners Submit the Long Form to FMCSA?
Many CDL drivers ask an important question after their DOT physical: “Does the examiner send my long form medical paperwork to FMCSA?” The short answer is no. The long form is part of your medical record and is not submitted to the Federal Motor Carrier Safety Administration.
What is the DOT long form?
The long form is officially called the Medical Examination Report. This document contains the detailed medical information from your DOT physical.
It typically includes information such as:
- Medical history
- Medications

- Blood pressure
- Vision and hearing testing
- Urinalysis results
- Medical examiner notes
Because this document contains personal health information, it is treated as part of your medical record.
What gets submitted to FMCSA?
After the DOT physical is completed, the medical examiner submits medical certification information electronically through the National Registry system.
This is what allows the driver’s medical certificate status to be updated with state licensing agencies.

- Expiration date of the medical card

Why the long form is not submitted
The long form contains detailed health information protected under medical privacy rules. Because of this, it is maintained in the medical examiner’s records rather than submitted to a federal database.
