Can I Get a DOT Physical Out of State While I’m Over the Road?

HealthRoute Compliance Team

You’re out of state, your DOT medical card is expiring, and you’re wondering if you can get a DOT physical anywhere. In most cases, yes—you can usually complete a DOT physical in any U.S. state as long as the exam is performed by a certified medical examiner. Here’s how it works, what to bring, and how to avoid CDL issues.

Quick note: This article is general education, not legal/medical advice. Requirements can vary by state and by your situation, but DOT medical exams generally follow FMCSA standards.

Short answer: Yes, you can usually get a DOT physical out of state

If you’re an over-the-road (OTR) driver, it’s common to renew your DOT medical card in a different state. The important part is choosing a provider who is a FMCSA-certified medical examiner (often listed on the National Registry).

Where should you go for an out-of-state DOT physical?

Look for clinics that regularly perform DOT physicals. Common options include:

  • Occupational health clinics
  • Urgent care centers (not all do DOT)
  • Some primary care offices (if they are certified)
  • Truck stop / travel center clinics (varies by location)
Pro tip: Before you drive there, ask one question: “Are you listed on the FMCSA National Registry as a certified medical examiner?” If the answer is unclear, pick a different clinic.

What to bring (especially if you’re out of state)

Bring these to avoid delays

  • Driver’s license
  • Glasses/contacts (if you wear them)
  • Medication list (name, dose, frequency)
  • Medical history (surgeries, diagnoses)
  • Specialist paperwork if applicable:
    • Cardiology clearance for AFib/heart conditions
    • CPAP compliance report for sleep apnea
    • Diabetes documentation (A1C/notes if requested)

How does the medical card get reported if you take it out of state?

This is where drivers get tripped up. The clinic may give you a printed card, but you still need to make sure your state driver’s license agency has the updated medical certificate (depending on your state’s process).

Two things can be true at the same time:

  • You passed your DOT exam and received a medical card
  • Your CDL can still be affected if your home state doesn’t receive the updated certificate on time
Best practice: After your out-of-state exam, ask the clinic: “Do you submit my DOT medical certificate electronically to my home state, or do I need to submit it myself?”

Can any doctor do a DOT physical?

Not every doctor can perform a DOT physical. The examiner must be listed as a certified medical examiner (National Registry). If the clinic is not certified, the exam may not count.

What if I’m in a hurry and my card is about to expire?

If you’re close to expiration, the biggest risks are:

  • Not being able to find a certified examiner fast enough
  • Missing paperwork (sleep apnea, heart conditions, diabetes)
  • Delays getting the updated certificate processed by your home state

Fast checklist before you go

  1. Call the clinic and confirm FMCSA-certified examiner
  2. Ask if they accept walk-ins and what hours DOT exams are performed
  3. Bring your med list and any specialist paperwork
  4. Ask how the medical certificate is submitted to your home state

What about owner-operators vs company drivers?

The exam standards are the same, but company drivers may also need to follow employer policies for scheduling and documentation. Owner-operators should plan early so they don’t lose driving time.
